In completing this form, the responsible person is agreeing to the adhere to the licensing requirements of the NOC. The responsible person must be in attendance when alcohol is being served and must be over the age of 25. I, the below named person, confirm that I have read and understood the terms and conditions included in this document and agree to conform to all procedures/law.. I understand that if the Designated Premises Supervisor/Event Manager /NOC Security feels that I am in breach of any part of the procedure the serving of alcohol must cease immediately. The decision of the DPS/Event Manager is absolute. Please list the relevant responsible person and their attendance date separated by a coma.

We are a wine trail sponsor and require approval to serve the approved wine as agreed with Diversified Ltd 4.30 pm - 6.30 pm.
